There has been do description of the WALANT technique for a dislocated distal radio-ulnar joint percutaneous pinning. We have described it in our case of a 19-year-old involved in a motor vehicle accident. A total of 10cc of WALANT solution was injected in the patient with 5cc on the ulnar side (including the periosteal region) and 5cc over the dorsal distal radio-ulnar joint. The pain score was 0/10 throughout the duration of the procedure which lasted for 16 minutes. The patient was slightly tachycardic due to anxiety prior to the wire insertion, mean heart rate, 105 beats per minute in anticipation of pain. However once the K-wire was inserted the mean heart rate was 81.4 beats per minute. Interestingly, the patient did not complain of any pain as the wire exited the final cortex.
